Flying with a dog - tips from people who have actually done it

Moving cross country and need to fly with my 15 lb cavalier king charles. Never done this before and honestly terrified.

Cabin or cargo? What airline? Any tips for keeping her calm? How far in advance do I need to book? All advice welcome.
